Skilled-Nursing Rehabilitation Services, “The Transitional Care Unit”
The Transitional Care Unit is a licensed skilled-nursing facility located on Alton Memorial Hospital’s first floor Hatch Wing.
If you qualify for extended care beyond your acute stay, our goal is to help you meet your rehabilitation goals so you may transition to the safest environment of living post-discharge.
Assistance with discharge plans is an ongoing process that begins the day you are admitted. The length of stay will depend upon your care needs and requirement of skilled nursing or skilled rehabilitation on a daily basis. As your level of care changes, your physician and our staff with discuss discharge plans with you. Participation in care rounds aids in communication with you, the team and the discharge process.
Skilled nursing services are provided 24 hours a day by a team of professional caregivers including RNs and CNAs.
The Rehabilitation experts of our Human Motion Institute devote their effort toward helping you achieve your optimal function. Physical, occupational and speech therapists provide rehabilitative therapy with you and your family.
Special events and therapeutic activities are a part of your stay on the Transitional Care Unit. A calendar of events is posted on the board in each room. We encourage our patients to participate in group activities, as well as one-on-one programs with the activity coordinator.
We have a family visiting room with computer access and cell-phone charging station, activity-dining room, therapy gyms, and free Wi-Fi.
As with all skilled-nursing units, your physician will not see you on a daily basis. Your physician will see you weekly. Nursing service provides you with daily rounds and assessments to keep your physician informed. If a change in your condition occurs, your physician will be contacted.
The Transitional Care Unit is located on the First Floor (one floor up from the Ground Floor) of Alton Memorial's Hatch Wing. Park in the Hatch Wing parking lot and use the Hatch Wing entrance (aqua awning). Either take the stairs just inside the entrance up one floor or use the Hatch Wing elevators across the hall from the entrance.
